Introduction
SMRU is a field station of the faculty of Tropical Medicine, Mahidol University, Bangkok, Thailand, and is part of the Mahidol-Oxford Research Unit (MORU) supported by the Wellcome Trust (UK).
The main objective of SMRU is to provide quality health care to the marginalized populations living on both sides of the Thai-Myanmar border in the Mae Sot area, Tak Province. This is achieved by the dual activities of research and humanitarian services, with an emphasis on maternal-child health and infectious diseases.
This combination of health care and research makes SMRU unique. The research agenda is driven by the questions we face in our clinical practice and whether clinical or laboratory based, our studies are designed to be of relevance to the population we serve.
Tasks and responsibilities:
STAFF SUPPORT / TRAINING
This is the heart of the clinic involvement for the doctors - the better this is done and the more the staff are equipped and empowered, the lighter the weight of the other responsibilities.
- Periodic (ideally at least once a week) formal teaching based on SMRU'S clinical guideline/drills. (check training log with Training team –different sites have different training needs – try to keep coordinated for the year)
- Timely, constructive, non-judgmental case reviews with the whole team when there are adverse events or near misses. Confidential enquiry always practiced with maternal deaths and unexpected perinatal deaths.
- Mentoring senior midwife (MW) and ensuring they are getting ongoing professional development
- Monitoring the progress of the staff who are potentially eligible for promotion. Discuss with senior MW to minimize bias and keep progress going for logbooks.
- Provide a letter to HR when the senior MW team and you are comfortable promoting a staff member
- Supervise MW logbook items when possible/necessary (eg. telephone consultation with doctor) and give feedback
- Assist with Periodic Advanced/Basic life support in obstetrics trainings, Implant trainings
- Updating Curriculum for MW course
- Updating guidelines when necessary e.g. PMTCT guideline; TB. Training is essential to this process by co-ordination with teams e.g. TB Doctor, Counselor Supervisor (and hopefully, soon, giving the trainings)
- Personalized support/mentorship for medics, particularly one staff who is a relatively ew medic.
CLINICAL
ANC:
- Support ANC team with holistic care: obstetric, medical, social and mental health
- Support collaboration and co-ordinate with ANC team and M-FUND, especially when insurance is not up taken by the patient
- Support ANC staff with acutely or chronically unwell pregnant women
- Support ANC staff for follow up of neonates never admitted to SCBU
- Support ANC staff in identifying and appropriately managing high risk pregnancies (eg twins, placenta previa)
- Support staff with overall functioning of the ANC especially if there is a training or staffing deficit endangering safety
- Supporting ultrasound staff by looking at abnormalities that they identify
- Training the ANC in new adjustments in the obstetrical ANC guidelines
- Ensuring patient information in the application is meaningful
- MSL specific – ensure ANC outreach function (consult with MSL team, logistics and IT)
Delivery
- Rounding with the staff on a regular basis.
- Managing high risk patients that are beyond the scope of the senior MW
- Make referral decisions in consultation with staff and manage these referrals by discussing them with the right people (referral person/doctor/referral hospital doctor) before sending. Then write the referral letter and make sure all other documentation is sent with it.
- Back up midwives for management of complicated labors (eg. augmentation, fetal distress)
- Support staff with overall functioning of the delivery room especially if they or you notice there is a training or staffing deficit endangering safety
- Ensuring patient information in the AMC application is meaningful
IPD/SCBU/OPD
- Rounding when there are cases inn IPD/SCBU
- Discussing complicated OPD (<3 m. babies and staff with SIMPLE problems) cases with medics
- Supporting medics in making decisions about how to triage and refer on to other health facility appropriately
- Supporting medics in counselling staff with complicated problems about where to seek care under SMRU’s policies.
Procedures:
- ECV (external; internal if multiple pregnancy)
- Tubal ligations/sterilization
- Dilation and curettage in a high-risk patient - the senior MW can almost always do these, but we try to do them when there is doctor present to backup
- Vacuum extraction
- Experience with Caesarean section
RESEARCH
- Protocol development/funding efforts
- Communication with potential PhD supervisors/collaborators
- EC submission
- Supporting with planning of colleagues’ academic work and cross-coverage
- Writing/editing/submission of papers
- Collaboration with research groups: TMEC Trop Med students, CMU
- Assist with smooth running of SMRU research studies whether they are clinical or laboratory based – agree on expectations
- Help with occasional requests for information from your site
- Do routine yearly exams for study or non-study (e.g. shoulder dystocia) infants
ADMIN/LOGISTIC
- Check and sign birth certificates - date, time, sex, birth weight, EGA, mark in logbook
- Help with coordination of certain expensive or controlled pharmacy items: sutures, ketamine, Anti-d IG, HBIG (when available)
- Help track referral outcomes and hospital visits as needed (including supporting referrals to other social or medical organizations as needed, supporting links with other orgs eg. KWO & KDHW in MSL)
- Participate in obstetric and physician meetings, sharing the task of writing the minutes and/or chairing
- Checking the lab forms for certain abnormal lab results and note them in the ANC app
SMRU doctors
- Take an active role in the doctor team to support development of each other and the unit
Qualifications:
- Medical degree (MD or DO) from an accredited medical school; advanced degree in clinical research or public health is a plus.
- Surgical track record in vacuum extraction and caesarean sections
- Valid medical license to practice as an Obstetrician in the state/country of employment.
- Prior experience in clinical research, including study design, data analysis, and publications, is preferred.
- Strong clinical skills, research methodology knowledge, excellent communication, and the ability to work independently and within a team.
- Ability to interpret complex data and contribute to evidence-based practices.
- Meticulous in-patient care and research documentation.
- Capable of addressing clinical challenges and designing research studies to investigate key questions.
- Demonstrates a proactive approach to advancing clinical practices through research.
